45 years old lady, diagnosed case of carcinoma stomach infiltrating liver with regional nodes. The patient underwent staging laparoscopy which was suggestive of non-metastatic disease (no disease elsewhere, peritoneal cytology was negative). The patient underwent neoadjuvant chemotherapy which was suggestive of partial response. The patient underwent total gastrectomy + en-bloc liver resection (segment 2 and segment 3) + D2 lymphadenectomy + total omentectomy and Roux-en-Y esophagojejunostomy anastomosis + jejuno-jejunostomy. The patient had lung collapse postoperatively which was managed conservatively and she was discharged uneventfully on 14^th post-operative day.
