40 years old gentleman, diagnosed case of adenocarcinoma stomach with regional nodes and single discrete retroperitoneal node in the inter-aortocaval region. The patient underwent staging laparoscopy which was suggestive of non-metastatic disease (no disease elsewhere, peritoneal cytology was negative). The patient underwent neoadjuvant chemotherapy which was showed partial response. The patient underwent total gastrectomy + D4 lymphadenectomy + nerve preserving retroperitoneal lymph node dissection + bursectomy + roux-en-Y esophago-jejunostomy + jejuno-jejunostomy. The post-operative period was uneventful and the patient was discharged uneventfully on 8^th post-operative day.
